A snapshot of our work in 2021

We enrolled 1,120 individuals in health insurance (Medicaid, Child Health Plus, Family Health Plus, financial assistance, and charity care) provided 1,650 individuals with post-enrollment assistance. We conducted healthcare outreach to 8,850 individuals.

We engaged our seniors with 3,200 wellness check-ins, 2,850 case assistance sessions(connection to SNAP, rental assistance, SSI, SSD, stimulus package, medicare/medicaid, SCRIE, DRIE), 416 supportive counseling sessions, and 41 senior support groups.

Registered 75 individuals to vote and also 150,000 texts were made (reminding people about the elections, voting, and checking polling stations).
